The answer could be something like:
"Moje adresa je U Rybníčku 14, Praha 2." (My address is U Rybníčku 14, Praha 2.)
Or "Bydlím na adrese U Rybníčku 14, Praha 2." (I live at the address U Rybníčku 14, Praha 2.)
